ከታች ያለውን የአባላት መመዝገቢያ ቅጽ በመሙላትና የዐመማ አባል በመሆን ከሚከተሉት በአንዱ ወይም ከዚያ በላይ በሆነ ረገድ አስተዋጽዖዎትን ያበርክቱ። 

• ለጠቅላላ ጉባኤ አባልነት ዕጩ ይሁኑ፣ 
• በተለያዩ ኮሚቴዎች በአባልነት ወይም በአመራር ለማገልገል ዕጩ ይሁኑ፣ እና/ወይም 
• የቦርድ አባላትን ለመምረጥ ወይም በአባልነት ለመመረጥ ዝቅተኛውን መስፈርት ያሟሉ።
